Claude Code 2.1.296 fixes BASH_ARGV0 permission bypass

Claude Code 2.1.296 fixes a Bash permission check bypass: some commands that first assign a value to the BASH_ARGV0 variable and then call it were previously auto-approved, and now they prompt for approval. This version also fixes issues such as PreToolUse hooks with denied calls in managed-settings, PostToolUse hooks not applying updatedMCPToolOutput, and --teleport opening an empty session when session history reading fails.
