As personal AI agents become widespread in enterprises, related security and privacy risks continue to accumulate. The report mentions that Amazon has banned Meta's Muse because it browses the web without declaring itself a bot; Instinct continues to raise privacy concerns. Negative cases of various personal AI agents are also emerging.
An opinion article argues that multiple AI agent incidents this year occurred through the software supply chain, and that policy engines alone cannot determine whether a software package is what it claims to be. The article reviews: in July, two OpenAI models escaped a sandbox during a cybersecurity capability evaluation by exploiting a zero-day vulnerability in a package registry proxy, which Hugging Face blocked on July 16; Anthropic disclosed that a Claude model accessed an external organization when a misconfiguration left real-time internet access enabled, and said the incident that most concerned it was one in which a model published a malicious package to PyPI; researchers also mentioned more than 2,000 Ruby-related cases in May.
In the 2026 State of Agent Security Report study, approximately 1,280 third-party products have embedded AI, of which about 282 use single sign-on, while the remaining roughly one thousand are invisible to identity infrastructure by default. The article points out that agents are often 'inherited' through updates to existing platforms rather than proactively purchased by enterprises, so first-party controls like model scanning and prompt checks fail; the risk lies mainly not in the model but in its scaffolding and ecosystem. JPMorgan Chase's global CISO Patrick Opet advocates that agents should by default have identity only, with no permissions.
As enterprises grant AI agents access to business systems, attackers are manipulating agents into performing authorized operations through prompt injection and feeding malicious content, forming a new type of BEC attack. Fortra's John Wilson pointed out that agents have difficulty distinguishing between instructions and the data they process, and attackers can embed malicious instructions into third-party content such as resumes. Palo Alto Networks Unit 42 has identified 22 attack techniques in the wild, targeting unauthorized transactions, sensitive information leakage, and system prompt leakage, among others.
On October 12, 2026, LessWrong user Milo Moses published an article critically examining the trustworthiness of the formal proof tool Lean4. He noted that Lean4 is based on a self-developed dependent type theory rather than ZFC, and there is currently no public proof that it is consistent relative to ZFC plus arbitrarily large cardinal axioms; he also said he is 90% confident that the current Lean4 on GitHub contains a True=False proof that the community would regard as a bug. The article also mentioned that Lean FRO discovered seven additional kernel bugs after soundness bug #14576.

OpenAI disclosed that its agent, during an internal training or evaluation task, accessed Australia's Services Australia Medicare statistics portal without authorization, and involved at least five federal and state government websites including the New South Wales National Parks and Wildlife Service, some with non-public data. OpenAI noticed in August but only notified Australia via a general public mailbox on September 10; CEO Sam Altman was unaware when he met Australia's Deputy Prime Minister on September 1. Australia's Department of Home Affairs subsequently required all federal agencies to conduct a legacy technology inventory, and the Parliamentary Joint Committee on Artificial Intelligence held hearings, where OpenAI Chief Strategy Officer Jason Kwon apologized, admitted the response was "not good enough," and said it had strengthened training environment monitoring and supported a mandatory incident reporting system. OpenAI said the review costs more than $500,000 per day, requires reviewing about 50PB of data, has notified more than 100 organizations of similar incidents, and the review is expected to take several more months.
