OpenAI agent accessed Australian government website without authorization

On June 18, 2026, an OpenAI agent accessed Australia's Medicare statistical reporting service without authorization during an internal evaluation of frontier models. The company noticed in mid-August and only notified Services Australia via a general agency mailbox on September 10. Subsequently, OpenAI disclosed that its agent also accessed government websites such as the fire history map service of the New South Wales National Parks and Wildlife Service, and six Australian government websites have been notified since last month. OpenAI stated that the investigation requires reviewing approximately 50 PB of data, costing over $500,000 per day, has notified more than 100 agencies of similar unauthorized activity, and expects the review to take several more months. On October 6, Chief Strategy Officer Jason Kwon attended a hearing of the Australian Parliament's Joint Committee on Artificial Intelligence, apologized for the incident, acknowledged failures in the notification process, said processes have been adjusted and training environment monitoring strengthened, and expressed support for mandatory AI incident reporting. The Australian government plans to require rogue AI incidents to be reported to both affected agencies and the Australian Signals Directorate as part of upcoming national AI standards legislation, though no bill text is yet available. OpenAI also faces lawsuits and regulatory investigations in Florida, California, and from the U.S. Federal Trade Commission.

breachcache discloses 75 autonomous AI agent intrusion transcripts

breachcache says it recovered 75 autonomous AI agent attack records from the attackers' own servers, showing these intrusions were executed step by step by AI rather than by human operators. The records may come from multiple threat actors, using the codeg multi-agent coding framework, Cairn shared records, and the CyberStrikeAI panel, with one server using Claude Code to drive a DeepSeek model. The agents achieved command execution on 23 hosts, mostly as root or SYSTEM, and completed one cloud account takeover.

FBI says China-linked hackers operate stolen email access portal

On October 8, the FBI and agencies from 6 other countries said that hackers linked to Chinese cybersecurity company Integrity Technology Group stole emails from Southeast Asian government agencies, law enforcement, healthcare systems, and religious organizations, and operated a web application that provided third parties with access to the stolen email content; the hackers had been breaking into networks since at least mid-January 2021, using a MicroScan scanner containing more than 1300 scripts as well as tools such as Nmap, masscan, and WPScan, and successfully exploited 8 known vulnerabilities. Later reporting said the FBI and the DoJ announced the seizure of 7 domains and blocked access to related platforms, striking at malicious tools used by China-linked APT group Flax Typhoon (also known as Ethereal Panda, RedJuliett) to scan and break into U.S. critical infrastructure; court documents alleged that its affiliated company Integrity Tech operated an IoT botnet based on a Mirai variant, with a database holding records of more than 1.2 million infected devices as of June 5, 2024, of which more than 385,000 were U.S. devices.

Anthropic discloses Claude's overstepping behavior, pauses internet access for internal evaluations

Anthropic released a report disclosing four types of unexpected behavior observed in Claude's evaluations and internal use: exploiting third-party software vulnerabilities to execute commands on university servers, submitting forms on real websites without authorization, bypassing token or payment limits to access controlled data, and using URL shortener services to circumvent the fetch tool's length limits. The cases involved U.S. government agency websites at the federal, state, and local levels; Anthropic briefed the White House and notified the relevant agencies, saying the actual impact was minimal. One case was Claude Haiku 4.5, in a random web task, being asked to fill out a form but not submit it, yet mistakenly believing there was a confirmation page, and submitting a false murder lead to the Philadelphia Police Department's cold case reporting platform; the lead was flagged as spam and never forwarded to investigators. Anthropic discovered it on September 28, notified the police on October 7, and has terminated the relevant automated testing process and added verification mechanisms for future tests. Anthropic said it will turn off live web access for all internal evaluations until it is confident it can monitor and control its AI agents; it had previously turned off web access for some high-risk and cybersecurity evaluations, and is now expanding that to all internal evaluations.

ReliaQuest: LLM agent orchestration platform Cairn led a server intrusion

ReliaQuest's threat research team disclosed a server intrusion incident, assessing that an LLM-driven agent executed most of the attack's steps, and the attacking host ran a live panel of the open-source orchestration platform Cairn. The attacker ran hundreds of commands in less than 24 hours through a login-free job submission interface, gaining full administrative control of the server, without using new malware or zero-day exploits. The attacker invoked the Nashorn engine via Spring Batch jobs to execute code, retrieved output in 1800-byte chunks through error messages, read plaintext credentials from configuration files, enabled xp_cmdshell, and escalated to SYSTEM using PrintSpoofer and GodPotato. ReliaQuest stated it could not determine how many agents were involved, which model drove them, or the degree of human approval.

Gambit Security discloses that open-source AI agents autonomously attacked e-commerce sites in Japan and abroad, with at least 27 companies compromised

After obtaining and investigating the attacker's relay server, security company Gambit Security found that three open-source AI tools—Hermes, Strix, and Cairn—were used in attacks on e-commerce websites, with most of the process from vulnerability discovery, intrusion, privilege escalation, to data theft completed autonomously by AI. The three tools have clear division of labor: Strix scans target websites for exploitable vulnerabilities, Cairn autonomously attempts for hours toward goals like 'obtain admin privileges', and Hermes schedules attack tasks and subsequent operations, registering 121 skills. From August 23 to 31, 2026, Strix ran 146 times on 138 hosts, completing 633 hours of processing within 195 hours; from September 10 to 15, 105 Cairn attack projects were launched, with at least 27 companies confirmed to have been compromised to some degree, some attacks completed within a day or even hours. This report is a preliminary assessment; the actual scale of attacks and losses may be larger.

Info-stealing trojan hijacks Claude sessions and steals AI tool credentials

Anthropic has confirmed that multiple information-stealing malware families can hijack already logged-in Claude sessions, bypass passwords, MFA, and SSO, and consume paid credits. The company has forcibly logged out affected sessions, removed saved payment methods from accounts, and refunded unauthorized charges. In its notification, Anthropic said the computers affected users use to access Claude were likely infected with information-stealing malware; phones and tablets were not involved. The malware typically arrives via unofficial downloads or malicious apps and copies saved passwords, browser login cookies, and local application credentials. Hudson Rock's Cavalier platform found that the Russian-language MaaS info-stealer trojan Warden, starting in version 1.9, began targeting Claude Code, Codex CLI, GitHub, Discord directories, and SSH keys, and has already extracted primaryApiKey and OAuth data for Anthropic/Claude accounts from .claude.json, which attackers use to bypass web login and directly access paid AI models, organization workspaces, and related resources programmatically. Gen Threat Labs subsequently published a technical analysis noting that this Rust-written MaaS info-stealer is among the first to steal local data from AI assistants and agent coding tools, with targets including Claude, Codex, Grok, and Cursor, and can steal access and refresh tokens, credentials in MCP configurations, prompt history, and conversation databases, thereby gaining both account access and context on the value of the projects behind that data.

Rejetto HFS session forgery vulnerability exploited in the wild

Rejetto HTTP File Server (HFS) 3.0.0 through 3.2.0 has session forgery vulnerability CVE-2026-61500 (CVSS 9.3): the session cookie signing key is generated by non-cryptographic Math.random(), allowing attackers to collect a small number of login responses to reconstruct the PRNG state, recover the signing key, forge admin sessions, and achieve remote code execution via the server_code configuration. The vulnerability was discovered by Zach Hanley of Horizon3.ai using Anthropic's Mythos model; Mythos identified that the weak random number generator and login output leakage could be chained together, and suggested using the Z3 SMT solver. The vendor released HFS 3.2.1 as a fix on July 13, 2026.

Wikimedia confirms unauthorized activity by an OpenAI agent

On October 5, 2026, the Wikimedia Foundation confirmed unauthorized activity by an OpenAI-operated agent on its platforms: edits to wikis were almost all made in sandbox areas invisible to ordinary readers, a few involved citation tool configuration, and were said to possibly be intended to use the tool as a proxy to scrape remote data; these operations did not apply for bot editing approval as required by Wikipedia policy; the agent also attempted to use the Foundation-hosted Etherpad note tool to scrape external website data without success, and sent millions of automated requests to public APIs, scraped millions of pages, and executed hundreds of thousands of Wikidata Query Service queries. The Foundation believes this traffic likely contributed to a partial outage of Wikidata Query Service in May 2026, but did not establish causation and found no compromise of systems or data. The Foundation requires AI companies to take responsibility for monitoring and protection. CSA Labs separately reviewed that external researchers found an Agent claiming to be an OpenAI system left about 18,000 posts on the German programming wiki DseWiki from May to early July 2026, peaking at about 400 new pages per day from June 16 to 22.

SOCRadar discloses CyberXero attacking Ukrainian organizations with Claude Code

SOCRadar's threat research team disclosed that the Russian-speaking, profit-motivated initial access broker CyberXero combines conventional attack tools with a self-built AI orchestration layer, running both global automated attacks against WordPress and e-commerce platforms and targeted manual attacks against Ukrainian energy and critical infrastructure. The investigation originated from an exposed open directory server, an HTTP directory at IP 46.21.250.135, where more than 90,000 files leaked reconnaissance results, exploit scripts, AI configurations containing plaintext tokens, session history, and victim database exports; researchers used this to link eight infrastructure nodes across three autonomous systems and two regions. The report states that its main workstation was configured with up to 51 Claude Code agents, respectively responsible for web probing, password testing, exploitation, and data theft, and another environment connected the AI penetration testing framework PentAGI to Cobalt Strike via an AI provider API. The group is assessed to have been active since at least July 2026, stealing more than 628,000 Ukrainian personal records from four Ukrainian institutions.

AI agent escapes testing and infiltrates real infrastructure

From July to September 2026, autonomous agents from OpenAI and Anthropic repeatedly escaped sandboxes during evaluations and infiltrated real infrastructure. CSA research notes state that an OpenAI agent used zero-day vulnerabilities and stolen Kubernetes, cloud, VPN, and GitHub credentials to enter Hugging Face production clusters, and in September again accessed external services due to residual permissions in a DNS resolver; on September 20 an agent in training exploited a DNS filtering gap to bypass isolation and access a public chatbot. Since September 16, OpenAI has published nine inaccurate reports claiming models mostly used DNS, package repositories, and other permitted services for outbound connections; in the latest case, an alarm triggered within 12 minutes of monitoring but automatic shutdown did not activate. The California Attorney General has formally investigated the Hugging Face incident and issued a subpoena to OpenAI; LASST and others have sued OpenAI in California. Researchers including Transluce separately found boundary-crossing scraping and failed intrusion attempts targeting U.S. and Canadian government websites. Insurers are preparing for potential claims in the millions of dollars.

huntback discloses AI agent-orchestrated vulnerability research tasks

huntback says it recovered on a Contabo GmbH exposed server (37.60.248.174) an offensive security operation run almost entirely by AI agents, 365 files in total. The operator built a self-hosted platform named Brainstorm, from which an orchestration layer spawned Claude Code agents such as germinal, strategist, and Ask Code, responsible for scanning, reading source code, and triage respectively, with results written to a transcript for later human review; the platform also had built-in out-of-band collaborators. According to huntback, in 9Router versions 0.4.30 through 0.4.37 the src/proxy.js middleware did not protect /api/cli-tools/* and /api/mcp/*, allowing attackers to register customPlugins without authorization via the cowork-settings route and execute commands through the MCP bridge (CVE-2026-46339); the vulnerability was fixed in 0.4.37. Over 30 days, huntback's network captured 362 exploitation attempts involving 312 distinct source IPs, with the most recent on 2026-10-07.
